| Membership Profile | Use this form to become a member of St. Matthew's.
If you're a member of another church this will begin the process of transferring your membership. |
| Baptismal Application | Use this form to apply to be baptized. This form should also be completed for baptism of your child. |
| Confirmation Application | Use this form to apply to be confirmed. This form should also be completed for confirmation of your child. |
| In Kind Donation | Use this form to record a donation of a product donated to the church. This is used in generating your tax deduction statement for tax purposes. |
| Check Request | Use this form to submit a reimbursement for expenses you have made for the church. |
